Our Financial Policy: 

For patients with participating insurance, applicable co-payments and deductibles will be collected from you prior to be seen by physicians.  If you have questions regarding your insurance coverage prior to your first visit, please call us. For non-insured patient or those with non-participating insurance, we will collect payment prior to your visit.

• Photo Identification

• Current Insurance Card

• Any insurance authorization that may be required

• A list of all medications you are currently taking

• A copy of test results with CD related to your visit

• A list of your questions and/or concerns
